Request a copy of your Allen Chiropractic records, or records from a previous provider.
Authorization to Release Records
Authorize us to share your records with a spouse, physician, insurer, or other party you specify.
What to Expect on Day One
Your first visit takes about 45–60 minutes. We start with a conversation — about your symptoms, your history, and what you're hoping to achieve. Then a physical assessment, and depending on your case, possibly a first treatment. You leave with a plan and a clear sense of next steps. No surprise fees, no high-pressure “care packages” — we're a small-town practice and we earn trust the old way.
